This Three-Way Agreement ("Agreement") is entered into by and between the patient ("Patient"), the healthcare provider ("Provider"), and the pharmacy ("Pharmacy"), collectively referred to as "the Parties." This Agreement outlines the responsibilities and expectations of all parties involved in the medication management process, ensuring effective communication, patient safety, and adherence to treatment plans.

Consent for Treatment and Information Sharing: The Patient consents to the prescribed treatment and authorizes the Provider and Pharmacy to share relevant health and medication information necessary for treatment purposes, following college guidelines to ensure privacy and confidentiality.

Medication Adherence: The Patient agrees to adhere to the prescribed medication regimen and to communicate any issues or side effects experienced to the Provider.

Privacy and Confidentiality: The Patient acknowledges their rights to privacy and confidentiality as protected under applicable laws and regulations.

Patient Care: The Provider agrees to offer high-quality, compassionate care tailored to the Patient's individual health needs.

Information Sharing and Privacy: The Provider commits to sharing necessary medication and health information with the Pharmacy while strictly following college guidelines to ensure the Patient's privacy and confidentiality are maintained.

Communication: The Provider agrees to maintain open lines of communication with both the Patient and the Pharmacy to facilitate effective treatment and address any concerns that may arise.

Medication Dispensing and Information: The Pharmacy agrees to accurately dispense medication as prescribed by the Provider, providing relevant information to the Patient regarding usage, side effects, and storage.

Privacy and Confidentiality: The Pharmacy commits to safeguarding the Patient's personal and health information, ensuring compliance with college guidelines and applicable privacy laws.

Communication: The Pharmacy agrees to communicate effectively with the Patient and Provider, ensuring that any discrepancies, issues, or clarifications regarding prescriptions are addressed promptly.

All Parties agree to adhere to the highest standards of privacy and confidentiality, following college guidelines and applicable laws to protect the Patient's personal and health information. Unauthorized disclosure of confidential information is strictly prohibited and subject to legal penalties.
